01

Retatrutide

Metabolic signalling

Open the medical brief

Retatrutide is a single engineered, long-acting peptide that activates GIP, GLP-1 and glucagon receptors. It is a calibrated multi-receptor pharmacology, not three drugs added together.

MECHANISM

GIP and GLP-1 receptor activity integrates glucose-dependent insulin secretion and appetite reduction. Glucagon-receptor activity may increase energy expenditure and lipid mobilisation while changing glycaemic and cardiovascular demands. Receptor balance, exposure and escalation are integral to the effect.

HUMAN EVIDENCE

In a 338-participant phase 2 obesity trial, mean weight change at week 48 ranged from −8.7% at 1 mg to −24.2% at 12 mg, versus −2.1% with placebo. A 98-participant MASLD substudy found large liver-fat reductions at week 24. Sponsor-reported phase 3 topline results remain labelled as such until full peer-reviewed reports are available.

CLINICAL BOUNDARY

Gastrointestinal events were dose-related and heart rate increased dose-dependently before declining. Scale weight does not resolve lean mass, nutritional adequacy, gallbladder or pancreatic symptoms, renal stress during severe gastrointestinal illness, or long-term outcomes.

02

Tirzepatide

Incretin medicine

Open the medical brief

Tirzepatide is a long-acting dual GIP/GLP-1 receptor agonist with a mature human development programme. Results belong to the exact construct, formulation and exposure studied.

MECHANISM

Integrated GIP and GLP-1 receptor signalling changes appetite, glucose-dependent insulin secretion, gastric physiology and energy intake. Dose, adherence, diet, resistance stimulus, sleep and concomitant therapy remain causal parts of the observed trajectory.

HUMAN EVIDENCE

SURMOUNT-1 randomised 2,539 adults. Mean weight change at week 72 was −15.0%, −19.5% and −20.9% with 5, 10 and 15 mg, versus −3.1% with placebo. Body-composition analysis estimated that approximately 75% of weight lost was fat mass and 25% lean mass on average.

CLINICAL BOUNDARY

Gastrointestinal adverse events and treatment discontinuation were more frequent during active treatment, particularly during escalation. A favourable average does not guarantee preserved function, adequate nutrition or an acceptable response for an individual.

Clinic Terrain editorial image for the BPC-157 clinical dossierCLINIC TERRAINRepair
03

BPC-157

Tissue repair questions

Open the medical brief

BPC-157 is a synthetic 15-amino-acid peptide derived from a sequence described in gastric-juice research. Identity, route and exposure must remain explicit.

MECHANISM

Experimental literature reports gastrointestinal integrity, angiogenic signalling, nitric-oxide pathways and effects in tendon, ligament and drug-injury models. The breadth is hypothesis-rich but does not create a universal repair mechanism.

HUMAN EVIDENCE

A small retrospective knee-pain series reported subjective improvement without randomisation, blinding, standardised function or systematic imaging. A 2025 intravenous pilot in two previously exposed adults detected no clinical or laboratory safety signal; two participants cannot establish efficacy or safety. No adequate prospective controlled clinical trial has yet established BPC-157 efficacy for tendon, ligament, muscle, gastrointestinal injury or recovery outcomes.

CLINICAL BOUNDARY

Human effect size, repeated-use tolerability and rare-event risk remain unresolved. Angiogenic and tissue-remodelling biology is not automatically favourable and requires context in proliferative disease, unexplained lesions and active bleeding.

TB-500 (Ac-LKKTETQ)

Recovery biology

Open the medical brief

In this monograph TB-500 denotes the N-terminally acetylated 17–23 fragment Ac-LKKTETQ. Full-length thymosin-β4 is a separate 43-residue entity. The terminal acetyl group is part of identity; LKKTETQ alone is incomplete.

MECHANISM

Full-length thymosin-β4 regulates actin dynamics, migration, inflammatory signalling, angiogenesis and wound biology. Those mechanisms cannot automatically be assigned to Ac-LKKTETQ.

HUMAN EVIDENCE

Controlled wound literature concerns defined full-length thymosin-β4 preparations and specific routes. Adequate human evidence for systemic Ac-LKKTETQ administration has not been established.

CLINICAL BOUNDARY

Evidence from full-length thymosin-β4, a preconditioned-cell study or another route is not direct evidence for systemic Ac-LKKTETQ. Sequence and terminal chemistry change degradation and target interaction.

CJC-1295 with DAC

Growth-hormone axis

Open the medical brief

Here CJC-1295 means the DAC construct: a tetrasubstituted GRF(1–29) analogue bearing a drug-affinity complex that binds albumin. A shorter-acting no-DAC construct is a different entity and cannot inherit its pharmacokinetics.

MECHANISM

CJC-1295 with DAC engages the GHRH receptor and prolongs stimulation of pulsatile growth-hormone secretion, with downstream IGF-1 elevation. Target engagement does not itself establish recovery, body-composition, sleep or longevity benefit.

HUMAN EVIDENCE

Human studies establish prolonged GH-axis engagement and preservation of pulsatility. They do not establish patient-relevant outcomes in recovery, body composition or healthy ageing.

CLINICAL BOUNDARY

An IGF-1 rise is a pharmacodynamic signal, not a clinical success. Oedema, paraesthesia, arthralgia, glycaemic deterioration, sleep-apnoea context and growth signalling remain biologically relevant boundaries.

Clinic Terrain editorial image for the Ipamorelin clinical dossierCLINIC TERRAINNeuroendocrine
06

Ipamorelin

Secretagogue pathway

Open the medical brief

Ipamorelin is a defined pentapeptide growth-hormone secretagogue. Its molecular identity and evidence are independent of every other GH-axis peptide.

MECHANISM

Ipamorelin activates the ghrelin receptor GHSR1a and produces growth-hormone release. Receptor selectivity defines mechanism but does not guarantee a meaningful clinical outcome.

HUMAN EVIDENCE

A phase 2 postoperative-ileus trial enrolled 117 patients. The primary endpoint was 25.3 hours with ipamorelin versus 32.6 hours with placebo and was not statistically significant (p=0.15).

CLINICAL BOUNDARY

The negative controlled trial is direct evidence that verified receptor pharmacology does not guarantee clinical translation for a chosen indication. It constrains that claim without negating GHSR1a engagement.

Clinic Terrain editorial image for the MOTS-c clinical dossierCLINIC TERRAINMitochondrial
07

MOTS-c

Mitochondrial signalling

Open the medical brief

MOTS-c is a 16-amino-acid peptide encoded within mitochondrial 12S rRNA. Endogenous MOTS-c and an administered preparation are related scientific questions, not interchangeable evidence.

MECHANISM

Experimental work connects MOTS-c with folate-cycle stress, de novo purine synthesis, AMPK activation, glucose utilisation and mitochondrial-to-nuclear signalling.

HUMAN EVIDENCE

Human studies associate circulating MOTS-c with metabolic status, PCOS, insulin resistance, muscle biology and exercise. These observations illuminate endogenous physiology but do not establish therapeutic efficacy.

CLINICAL BOUNDARY

At the evidence cutoff, no adequate randomised human efficacy trial of native exogenous MOTS-c was identified. Association and exercise response cannot substitute for administered-product exposure.

Clinic Terrain editorial image for the Thymosin alpha-1 clinical dossierCLINIC TERRAINImmune
08

Thymosin alpha-1

Immune modulation

Open the medical brief

Thymosin alpha-1 is a defined 28-amino-acid thymic peptide with a larger human literature than most non-incretin peptides in this collection.

MECHANISM

The mechanistic programme includes dendritic-cell function, Toll-like receptor pathways, T-cell maturation and immune restoration. ‘Immune boosting’ is too imprecise to represent this biology.

HUMAN EVIDENCE

Trials in chronic viral hepatitis and oncology have produced heterogeneous results. A large phase 3 sepsis trial randomised 1,106 patients: 28-day mortality was 23.4% with thymosin alpha-1 and 24.1% with placebo (HR 0.99; p=0.93).

CLINICAL BOUNDARY

The neutral sepsis result is decisive counterevidence to a broad mortality claim in unselected sepsis. It does not erase molecular mechanism or answer every narrower immune phenotype.

Clinic Terrain editorial image for the Selank clinical dossierCLINIC TERRAINNeuroregulatory
09

Selank

Neuroimmune questions

Open the medical brief

Selank is a synthetic heptapeptide related to tuftsin. The clinically reported intranasal route and any proposed parenteral route are different exposure experiments.

MECHANISM

Proposed actions include GABAergic modulation, neurotrophic pathways and immune–neural communication. Much of the causal programme remains preclinical.

HUMAN EVIDENCE

A comparative report in 60 patients with anxiety and somatoform disorders described anxiolytic and mild cognitive effects relative to phenazepam, but accessible reporting leaves randomisation, blinding, allocation and endpoint hierarchy insufficiently resolved.

CLINICAL BOUNDARY

Small human reports establish administration and a signal worth studying; they do not establish broad anxiolysis, cognitive enhancement or injectable efficacy.

Clinic Terrain editorial image for the Semax clinical dossierCLINIC TERRAINNeuroregulatory
10

Semax

Neurotrophic signalling

Open the medical brief

Semax is the defined heptapeptide Met-Glu-His-Phe-Pro-Gly-Pro, derived from an ACTH(4–7) motif with a Pro-Gly-Pro extension. It is not ‘Samax’ and it is not a combination.

MECHANISM

Preclinical studies report BDNF/TrkB and neuroplasticity-related changes. Network or molecular modulation is target engagement, not proof of cognitive or clinical benefit.

HUMAN EVIDENCE

A small resting-state fMRI study included 24 healthy participants and reported network-level change. Small or methodologically limited stroke cohorts have reported clinical signals in treatment contexts that require comparison with modern stroke care.

CLINICAL BOUNDARY

Imaging change does not establish cognitive benefit, and historical non-equivalent controls do not support a universal neuroprotection claim. Long-term and injectable safety remain unresolved.

Clinic Terrain editorial image for the GHK-Cu clinical dossierCLINIC TERRAINTissue & skin
11

GHK-Cu

Copper peptide biology

Open the medical brief

GHK is glycyl-L-histidyl-L-lysine; GHK-Cu is its Cu(II) complex. GHK, GHK-Cu, free copper and mixtures are not biologically interchangeable. Stoichiometry is part of identity.

MECHANISM

The mechanistic programme includes copper binding, extracellular-matrix regulation, fibroblast collagen synthesis, wound biology and gene-expression effects under defined experimental conditions.

HUMAN EVIDENCE

Topical cosmetic literature reports signals but products vary. In a 13-participant randomised study after CO₂ laser resurfacing, topical copper tripeptide did not improve objective erythema, wrinkles or overall skin quality, although satisfaction differed.

CLINICAL BOUNDARY

The controlled topical result constrains the leap from fibroblast biology to guaranteed visible rejuvenation. Topical evidence cannot establish systemic regeneration or injectable benefit.

Peptide identity gate

There is no generic peptide mechanism.

A peptide protocol becomes scientifically interpretable only when the administered molecular entity can be connected to its own target, exposure, physiology and response.

01 · IDENTITY

Sequence and construct

Primary sequence, terminal chemistry, conjugation, metal coordination and formulation define the entity.

02 · TARGET

Molecular interaction

Receptor agonism, binding or signalling must belong to that exact entity, not a parent peptide, fragment or commercial nickname.

03 · REACHABILITY

Route and pharmacology

Proteolysis, albumin binding, clearance and tissue access determine whether the proposed mechanism can be reached.

04 · RESPONSE

Individual falsification

A predefined functional or biological measure determines whether to continue, modify or stop the pathway.

01

Know the molecule before discussing the protocol

Peptides are not one treatment category. Clinic Terrain maintains molecule-specific monographs that separate sequence, targets, exposure, human evidence, counterevidence and unresolved questions. Retatrutide, BPC-157, GHK-Cu, MOTS-c and Selank cannot borrow one another’s mechanism or certainty.

02

Start with the patient, then accept or refuse the molecule

The objective, history, medication, diagnosis, laboratory context and previous response are assembled before the physician decides whether a peptide is coherent. The outcome may be a protocol, a different starting point or a documented refusal.

03

Two possible supply routes; one release standard

A finished presentation may be purchased through a documented manufacturer or supplier. A locally prepared presentation may be considered only when the responsible authorised entity, raw-material source, formulation, preparation record and release documentation are all identifiable. Clinic Terrain does not treat local preparation, an invoice or a supplier relationship as proof of molecular quality.

04

Match the exact product to the exact evidence

Before any final supply is represented as traced, the proposed molecule, formulation, strength, presentation and current lot must be connected to the available analytical record, receipt and storage trail. Identity or purity for another molecule, format or historic lot does not transfer.

05

Make the response change the next decision

Baseline, product identity, dose, adherence, tolerance, symptoms, biomarkers and functional response remain in one longitudinal record. The physician continues, adapts or stops from observed trajectory rather than from a fixed commercial calendar.
